Occupational Therapist-Job Opportunity

Our clinic is looking for a dedicated Occupational Therapist to join our team! We are looking for someone who has previous experience conducting in-home and Form 1 assessments. This position is contract based with pay ranging from $300-$400 per assessment.

Responsibilities:
– Conduct patient Inhome Assessments with a FORM 1.
– Provide occupational therapy interventions to enhance patients’ daily living skills and independence
– Collaborate with healthcare professionals to ensure comprehensive patient care
– Maintain accurate patient records and documentation
– Educate patients and their families on therapeutic techniques and exercises

Qualifications:
– Previous experience assessing and providing reports ( Form1)

License/Certifications:
Licensed by the College of Occupational Therapists of Ontario (COTO)

Experience:
OT: 1 year (required)
