Qualicare location offers home care services that focus on providing the best quality of life for our clients and peace of mind for their families.Job SummaryThe Care Coordinator is accountable and responsible for evaluating client needs, overseeing and monitoring the care delivered, developing an appropriate plan of care, and ensuring the competency of caregivers. The Care Coordinator commits to participate in establishing and implementing the highest standards of care.The Care Coordinator provides leadership and direction in all aspects of client care. She/he functions and participates in the development, implementation, monitoring, and evaluation of client programs and services while upholding Qualicare brand standards. This individual will assist in the delivery of home care services including client assessments, health education/counseling, and follow-up care.ResponsibilitiesConsults with supervisors and staff regarding clients, families, and service programsEnsures compliance with standards of careConducts in-home assessments and creates care plans for clientsConsults with physicians and other care providers related to client care servicesPerforms supervisory visits to clients on a regular basisConducts employee selection process and supervises the overall performance of staffParticipates in quality assurance and organization risk management programsCommunity MarketingQualificationsCurrent certificate of competence and in good standingStrong clinical skillsSelf-directing with the ability to work with little direct supervisionExcellent customer service skillsExperience with direct employee supervision
